Wisdom Teeth: To Remove or Not?
Impacted wisdom teeth don't always need removal. Here's how to decide — case by case.

The removal question
Unlike the old default "remove all wisdom teeth at 18," modern evidence supports case-by-case decisions. Some wisdom teeth come in perfectly and function for a lifetime; others cause problems and should be removed.
Remove when
- Partially erupted with recurrent gum infection (pericoronitis)
- Decay in the wisdom tooth or adjacent second molar
- Cysts or tumours around the impacted tooth (rare but serious)
- Ongoing pain or pressure symptoms
- Orthodontic treatment requiring space
- Damage to adjacent second molar (root resorption)
- Active gum disease around the tooth
Monitor (don't remove) when
- Fully erupted, functional, cleanable
- Deep bony impaction with no current or likely symptoms
- Patient is elderly and surgery risk outweighs benefit
- Single isolated symptom that resolves with conservative care
The age factor
- 17–25: easier healing, flexible roots, best surgical window
- 26–40: still straightforward
- 40+: roots more formed, bone denser, slightly more complex recovery
- 60+: significantly higher surgical risk; usually only removed if causing real problems
Types of extraction
Fully erupted, routine
Same as any other extraction. 15–30 min, local anaesthetic, minimal recovery.

Soft-tissue impacted
Tooth through bone but gum partially covers it. Small gum incision, then extraction.
Cost in Dubai: AED 900–1,500
Partial bony impaction
Partly in bone. Gum flap, minor bone removal, extraction.
Cost in Dubai: AED 1,500–2,500
Complete bony impaction / horizontal
Fully in bone, often on its side. Significant surgical procedure.
Cost in Dubai: AED 2,500–4,500
Recovery by extraction type
- Fully erupted: 2–3 days
- Soft-tissue impacted: 3–5 days
- Partial bony: 5–7 days
- Complete bony / horizontal: 7–14 days
Sedation options
For deeper impactions, IV sedation is available at most Dubai clinics. You're drowsy and relaxed but safely awake. Recovery takes a few hours and you need a ride home.
Cost for IV sedation: AED 1,500–3,000 additional.
Common complications
- Dry socket: 5–10% in lower wisdom teeth, higher in smokers
- Paresthesia (temporary numbness): rare, usually resolves over 3–6 months
- Sinus communication (upper wisdom teeth close to sinus): managed with careful closure
- Swelling and bruising: normal, peaks at 48–72 hours
What to eat
Days 1–3: soup, yoghurt, smoothies (no straw), mashed avocado, scrambled eggs, well-cooked pasta Days 4–7: softer solids on the opposite side Week 2+: normal diet on the opposite side; be cautious on the extraction side for another week
