What TMJ actually means
The temporomandibular joint (TMJ) is the hinge where your lower jaw meets your skull. TMJ disorders (TMD) cover a range of problems: muscle overuse, disc displacement within the joint, arthritis, or a combination.
The typical symptoms
- Jaw clicking or popping when opening
- Pain in front of the ear
- Headaches at the temples
- Ear fullness, ringing, or pressure without ear infection
- Limited mouth opening

- Morning jaw stiffness
The main causes
Bruxism (grinding / clenching)
The #1 cause in most adults. Daytime clenching from stress, nighttime grinding, or both.
Disc displacement
The cushioning disc within the joint slips forward, creating clicking and sometimes locking.
Arthritis of the joint
Age-related or rheumatoid. Can be genuinely painful, especially chewing tough foods.
Bite problems
Uneven bite can overwork certain muscles.
Postural / neck issues

Trauma
Direct blow, whiplash, or prolonged mouth opening (dental procedures, intubation).

- Gentle jaw stretches
- Avoid gum, hard candy, wide yawning
- Over-the-counter ibuprofen short-term
- Reduce caffeine, manage stress triggers
Tier 2 — Professional care
- Custom nightguard (or day splint) — often the single most effective intervention for grinders
- Physiotherapy (jaw massage, posture, exercise)
- Stress management, CBT for bruxism
Tier 3 — Targeted medical
- Botox into masseter muscles — proven effective for muscle-dominant TMD; 3–4 month duration
- Trigger point injections
- Short course of muscle relaxants
Tier 4 — Specialist
- TMJ arthroscopy (if disc displacement doesn't respond to other treatment)
- Joint injections
- Orthognathic surgery (rare, severe structural cases)
What usually doesn't help
- Bite-grinding to "correct" tooth contacts as first-line treatment (often unnecessary)
- Permanent orthodontic changes until symptoms are stable
- Aggressive jaw surgery in the absence of clear structural pathology
